24/7 on-call nursing support
Reach a hospice nurse by phone at any hour, including nights, weekends and holidays. When it is medically necessary, the on-call nurse can arrange a visit.

Nursing and hospice aide care
Skilled nurses manage pain and symptoms. Certified nurse aides help with bathing, grooming and personal care, on visits set by the plan of care.

Respite care
Up to five days in a Medicare-approved facility, so the family caregiver can rest and recover.

How care works
Four levels of care, so support can change as needs change
Medicare defines four levels of hospice care. Most patients receive routine home care, and the team steps up to a higher level when symptoms or the family’s needs call for it.

Routine home care
Scheduled visits from the hospice team, wherever the patient lives. Most hospice care happens at this level.

Continuous home care
Additional nursing care at home during a qualifying medical crisis, when medically necessary and in line with Medicare guidelines.

General inpatient care
A short stay in a facility when pain or symptoms cannot be managed at home.

Respite care
Up to five days in a Medicare-approved facility so the family caregiver can rest.

Caring for a loved one at home is demanding, day and night. Respite care gives family caregivers a rest while the patient stays in a Medicare-approved facility, cared for by trained staff.
- The hospice plan of care continues during the stay, including medications and symptom management.
- Our team arranges the stay and the transport when it meets medical necessity.
- It can be used more than once, on an occasional basis.
Asking for respite is not a sign of failure. Caregivers who rest are better able to keep caring.

No. You or your loved one can contact Elegant Hospice directly. Our team will guide you through the evaluation and coordinate the physician certification needed to determine hospice eligibility.
Who pays for hospice care?
Hospice care is covered by Medicare Part A, Texas Medicaid and many private insurance plans for eligible patients. There are no out-of-pocket costs for hospice-covered medications, medical supplies, equipment and services related to the terminal illness.
Where is hospice care provided?
Wherever the patient lives: their own home, a family member’s home, an assisted living community or a nursing home. We serve San Antonio and the surrounding area.
What if we need help at night or on the weekend?
Elegant Hospice provides 24/7 on-call nursing support, including nights, weekends and holidays. When medically necessary, an on-call nurse can arrange an in-person visit and coordinate with the hospice physician.
Does choosing hospice mean giving up?
No. Choosing hospice does not mean giving up hope. It means shifting the focus from curing an illness to providing comfort, dignity and quality of life.

Call or fax the referral and we will contact the patient and family, then keep you informed. You can stay involved as the patient’s attending physician.
